Several cast members are set to return. Martin Henderson will both act in and direct an episode. Austin Nicholas will reprise his role as Dr. Eli in a recurring capacity, with showrunner Patrick Sean Smith telling the Daily Mirror that Eli would bring a 'new energy' to the show, though he won't be caught up in another love triangle. Codey Kearsley will return as Clay, and Matty Finochio is expected to return as Tony, scheduling permitting. However, Marco Grazzini, Sara Canning, Lauren Hammersley, Rachel Drance, and Nicholas Coombe are not expected to return.

Season eight will feature a four-month time jump, bypassing the immediate medical aftermath of Mel and Jack's adopted daughter's congenital heart defect. Viewers will learn the outcome of Brady's motorcycle accident. Denny will navigate the medical school application process after his MCAT results, while Lizzy is heading back to work. Brie will assist Clay in searching for his sister, and a new character, Layla, may be introduced as Clay's sister, though no casting has been announced.
