Ieuan Williams, 15, underwent a brace-fitting procedure at Royal Gwent Hospital in Newport on February 25. He is among 21 patients affected by a mistake involving unsterilized medical instruments at the hospital. The mistake occurred on February 25 and 26.

The unsterilized instruments used in Ieuan's procedure were pliers and a mirror. The instruments had been disinfected but not placed in an autoclave for sterilization due to human error, according to the health board. Three weeks after the procedure, a man wearing scrubs and a surgical mask delivered a letter to Ieuan's family asking them to contact the health board urgently.

Aftonbladet reports that the letter was from the director of the Aneurin Bevan University Health Board. The affected patients are at risk of contracting blood-borne diseases such as HIV, Hepatitis B, and Hepatitis C. The health board says the risk of contracting these diseases is very low.

Ieuan will undergo testing over a six-month period before receiving the all-clear. Aftonbladet reports that the mistake was discovered on February 27 during routine checks. Patients were informed on March 19.

Karen and Lee Williams accuse the health board of a cover-up, alleging that the health board only informed patients after a whistleblower contacted a local news website. The identity of the whistleblower and the exact information they provided remain unknown. Karen Williams said, 'If it wasn’t for the whistleblower, would we have ever known?

They need to be more transparent with us. ' She added, 'I couldn't process it. ' Ieuan Williams said, 'I was quite scared at first.

I'm going to have to go back and forth to the hospital for six months. ' Local politicians have condemned the health board's handling of the error. ' Peter Fox, health spokesman for the Welsh Conservatives, said it was a serious breach of care and 'an apology alone is not good enough.

' It is not known what disciplinary actions, if any, have been taken against the staff responsible, or what measures the health board has implemented to prevent similar incidents in the future. The health board has not released information on how many other patients have been tested so far or what the results are.
