Region Gävleborg in Sweden has introduced a self-monitoring program for patients with chronic diseases, starting with heart failure. The initiative began at heart clinics in Gävle and Hudiksvall. Patients can voluntarily record health metrics like weight and blood pressure, and report symptoms through the Min vård Gävleborg app, with the data monitored by healthcare providers.
The program aims to detect health changes early, potentially preventing hospitalizations and offering better support between visits. It is a complement to regular care, not a replacement, and will be implemented gradually for selected patient groups. Jennie Forsblom, chair of the health and medical care committee, called it an important step for patient support, while doctor Anders Lundberg emphasized its role in improving follow-up and patient safety.
