Luleå Municipality has reportedly self-reported to the Swedish Health and Social Care Inspectorate (Inspektionen för vård och omsorg, Ivo) after a patient discharged from Sunderby Hospital went without home care services for three weeks. According to local media reports, the incident was discovered when the patient was readmitted to the hospital. The municipality stated that the failure occurred because the patient's absence was not deregistered in the planning system when they were discharged from Sunderby Hospital.
As a result, home care services were not resumed. The municipality has filed the report under Lex Sarah, which requires healthcare providers to report serious deficiencies in care. ' The consequences for the affected individual are reportedly still being investigated.
We are now strengthening our routines so that we can even better ensure continuity and quality in care.
The case has been submitted to the Health and Social Care Inspectorate for review.
