IVO has decided to close the case regarding a Lex Sarah report from the elderly committee in Jönköpings kommun concerning misconduct at Vilhelmsrogården's elderly care home. The inspectorate assesses that the municipality has fulfilled its obligations and taken necessary measures. IVO cites as reason for the decision that the municipality has done what is required: investigated the events according to Lex Sarah, identified causes, and implemented both immediate and long-term measures. The municipality's measures include strengthened competence development for employees, clearer routines for care and work with elderly care's value base, and ongoing follow-up to prevent similar deficiencies from arising again. Jönköpings kommun continues its work to strengthen the quality and safety for residents.

In a separate development, Region Dalarna is making a Lex Maria report regarding an elderly man who received a delayed diagnosis of prostate cancer. A man in his 70s sought care at a health center in southern Dalarna for severe urination problems, saw a temporary doctor, tried a medication, and was referred to a urology specialist. The PSA blood test, used for early detection of prostate cancer, was normal, and the urologist found no signs of cancer in the prostate gland. In the referral response, the urologist recommended annual follow-up with PSA tests and examination of the prostate, considering the man's brother had prostate cancer. The referral response was signed by another temporary doctor who did not take any further action. Four years later, the man sought the health center for other reasons and also mentioned increased urination problems. A new PSA test was taken which this time was elevated. A new examination by a urologist showed prostate cancer, and the man received treatment with radiation and chemotherapy. The patient did not perceive that he was recommended annual check-ups, and the health center did not call him for follow-up visits. Lack of doctor continuity at the health center may have contributed to the incident. The missed check-ups may have affected the disease's prognosis and the patient's quality of life. The operation has taken measures, including a new routine for how referral responses are received.

Region Värmland is making a report to IVO after a patient received incorrect administration of medication. The patient is a multi-sick patient with impaired swallowing ability, had suffered several pneumonias due to stomach contents entering the lungs, and had a PEG tube inserted for feeding. During hospitalization for continued escalation of tube feeding, the patient also received nutrition via a central venous catheter. The next day, the patient was affected and doctors were called. It turned out that medication intended to be given via the PEG tube had been administered into the venous catheter. The patient was treated in the intensive care unit but then improved and could be moved to a ward.

According to SVT Mittnytt, Kramfors kommun has extensive deficiencies in handling cases concerning children and youth. The criticism includes deficiencies in several areas: failing to check that residences have correct permits for the child's target group and that family homes are not run professionally, missing to collect references from other municipalities, not following legal requirements for deciding on investigations or completing investigations within time limits, not following up placements to ensure children talk with their social secretary at least every half year, and lacking work to review journals and documentation. The municipality explains the deficiencies with high staff turnover, lack of continuity and leadership, and deficiencies in competence and experience levels among social secretaries. Kramfors kommun now must report to IVO how it will address the deficiencies.

In another case reported by SVT Östergötland, IVO states in its assessment that there is a clear and serious risk to patients' health, safety, and life. The review of the operation has shown significant deficiencies, meaning the company lacks basic conditions to offer safe and secure care and treatment. According to SVT Östergötland, Psykiatrispecialisterna is appealing the decision to the Administrative Court and has submitted an application for inhibition, meaning the decision should not apply for the time being or be referred back to IVO.

According to Region Norrbotten Press, Avesta kommun has submitted a report in accordance with Lex Sarah to IVO. An internal investigation has been initiated in connection with this.
